evaluation to identify individual concerns and needs.

Obtain, screen, and oversee in-home
and auxiliary services.

Make referrals to appropriate
geriatric professionals and specialists as needed.

Provide counseling and support.

Offer individual and family therapy.

Act as a liaison between client,
family members, care providers and community resources
to facilitate communication and optimize service delivery.

Evaluate long term housing needs.

Assist in relocation if necessary.

Provide consumer education and
advocacy.

Be available to client and families
on-call 24 hours.
