- Obtain, screen, and oversee in-home and auxiliary
services.
- Referrals made to appropriate geriatric professionals
and specialists as needed. Provide counseling and support.
Offer individual and family therapy.
- Act as a liaison between the client, family members,
care providers, and community resources to facilitate
communication and optimize service delivery.
- Evaluate long-term housing needs. Assist in relocation
if necessary.
- Provide consumer education and advocacy.
- Provide crisis management.
- 24 hour, on-call availability to client and families.
